=== Routes of administration === Alternative routes of administration, such as insufflation, rectal administration, intravenous, intramuscular, inhalational aerosol, transdermal, or sublingual, avoid or partially avoid the first pass effect because they allow drugs to be absorbed directly into the systemic circulation. Drugs with high first pass effect typically have a considerably higher oral dose than sublingual or parenteral dose. There is marked individual variation in the oral dose due to differences in the extent of first-pass metabolism, frequently among several other factors. Oral bioavailability of many vulnerable drugs appears to be increased in patients with compromised liver function. Bioavailability is also increased if another drug competing for first-pass metabolism enzymes is given concurrently (e.g., propranolol and chlorpromazine).

It is the over-expression of efflux pump proteins observed in resistant cancer cells that allows them to produce these pumps in high quantities and/or increase their overall activity compared to normal, sensitive cells. A specific protein and member of the ATP-binding cassette (ABC) transporter family known as p-glycoprotein (P-gp), has been suggested to play a major role in the development of resistance in cancer cells by promoting drug efflux. Research on P-gp and other related efflux pumps is currently ongoing, but studies have suggested that they have the capacity to transport various types of chemotherapeutic agents out of cancer cells resulting in a significantly reduced bioavailability of such drugs, and ultimately failed chemotherapy treatments. The inhibition of efflux pumps has been suggested as a promising therapeutic target to overcome this mechanism of resistance in cancer cells, aiming to both restore chemotherapeutic drug bioavailability within the cells, yet avoid deregulating normal functioning pumps and reduce undesired adverse effects. Conversely, toxic drug interactions have posed a major obstacle to successfully developing efflux pump inhibitors and highlights the difficulty of targeted chemotherapeutic drug formulation. Many proteins involved in intracellular transport processes have been identified as fundamental contributors of resistance by impairing chemotherapeutic drug-target interactions.
=== Miniature time of flight === Some researchers are successful in designing a series of miniature TOF mass analyzers. Cotter at Johns Hopkins University used a pulsed extraction in linear time of flight mass analyzer and the ions are accelerated to with higher energy of 12 keV to enable detection of high-mass. The group achieved resolutions of 1/1200 and 1/600 at m/z 4500 and 12000 respectively. This mini analyzer can measure 66k Da proteins, mixtures of oligonucleotides, and biological spores. Verbeck at University of North Texas, created a mini-TOF based on reflectron TOF with a microelectromechanical system technology. To overcome the low resolution of short flight tube, the effective ion travelling path length is extended by moving ions back and forth in periods of time. The system used a 5-cm endcap reflectron TOF with higher-order kinetic energy focusing to analyze the ions with m/z exceeding 60,000. Ecelberger, a senior professional staff scientist in the Sensor Science Group of the Research and Technology Development Center at APL also developed a suitcase TOF incorporated with matrix-assisted laser desorption/ionization MALDI. The suitcase TOF was tested by scientists from U.S. Army Soldier and Biological Chemical Command. The samples are biological toxins and chemical agents with the mass range from a few hundred daltons to over 60 kDa. The Suitcase TOF was referenced with a commercial TOFMS for the same experiments. Both instruments can detect all but a few compounds with very encouraging results.
